Change of Address

Change of Address or Guardianship

ANY change of address or guardianship requires documentation to be immediately provided to Central Registration located at Mattituck Jr/Sr High School. 

Failure to do so may result in the student's immediate withdrawal from the Mattituck-Cutchogue School District.

No change in a student's transportation, due to a change of address, will be honored until the appropriate documentation is received by Central Registration.

Photo Identification of Parent Or Guardian- need one:

  • NYS Drivers License
  • NYS Identification Card
  • Military Identification
  • Passport
  • Resident Alien Card

Proof of Residence:

Home Owner – need two

  • Deed, current mortgage statement or tax bill

AND

  • Current utility bill (cable, LIPA, fuel, water)
  • Photo Identification (drivers license, military ID, passport)
  • Current checking, savings or major credit card account statement
  • *Note: Telephone bills are not acceptable

Renter – need the following

  • Notarized Lease (original)
  • Rental Registration AFFIDAVIT & copy of landlords tax bill or deed

AND

  • Current utility bill (cable, LIPA, fuel, water)
  • Photo Identification (drivers license, military ID, passport)
  • *Note: Telephone bills are not acceptable
