Health Announcements

Attention parents of students entering the 7th and 12th grades for the 2017-2018 school year:  New York State has changed the Meningococcal immunization requirements for students entering the 12th grade.  Please check with your child's healthcare provider to be sure that Meningococcal vaccines are up-to-date.

Whenever your child receives any new vaccines, please make sure that the Health Office receives and updated copy of your child's immunization records.

New York State requires all new students and those students entering grades 7 and 10 to have a physical exam (health appraisal).  If your child will be a new student or will enter these grades in September 2017, you must submit a physical exam that is done withing the past year to the Health Office.  May we suggest keeping a copy of your child's physical exam for your own record.

Any students wishing to participate in sports are required to have a current physical exam as well as a completed Family ID registration on file.  Physical exam forms are available on the website or in the health office. For each sport season, registration via Family ID needs to be completed.  Registration opens one month before the start of each sports season.  A student will not be permitted to participate or practice a sport unless registration is completed.

Please contact the High School Health Office at (631) 298-8460 x3271 should you have any questions.
